Chronic Care Staffing, LLC (“CCS”) was founded in 2015, the same year Medicare’s Chronic Care Management CPT 99490 / G0511 program began.  Our headquarters are in Charleston, SC. Chronic Care Staffing has compiled an array of clients ranging from small practices to large health systems.  Our clinical team has developed industry leading documentation, population health initiatives and Value Based Reimbursement (VBR) calling protocols to enhance future reimbursement.  Here are some highlights:
